Agreed. I am guessing that their target group for the study was anyone who identified as “catholic” even if they attend Mass maybe twice a year.
#1 and #3 – I think you are correct but in my experience there is a shocking number of Catholics who do go to Church but who believe any number of things well outside the Catechism – or anything recognizably Christian (such as abortion, gay marriage, and so on). There is a schism within the Church and has been since V2, just no one dares talk about it. The parish I used to be a member of had many of these people, and they received encouragement from laity involved in parish administration and adult catechesis (I now attend a traditional Latin Mass parish). A lot of the worst apostates you have in TEC are former Catholics and brought a lot of their bad ideas with them. Sadly for us, plenty of their fellow travelers remained.
